I've attached a sample order form as well as a document featuring the products I offer. Order envelopes will be provided on Picture Day. I prefer parents use the envelopes I provide so that payments can be inserted inside. I do understand there are times when parents need to print the order form in advance. Payment is due when photos are taken. I accept cash, check, PayPal and Venmo. If you use one of the apps to send payment, please make sure you add your team name and swimmer's name in the "Note" section. My PayPal and Venmo information is noted on the top of the order form.An order form must be completed. Please DO NOT ORDER THROUGH A PAYMENT APP.
